Osteoporosis & Dental Implants
Osteoporosis is a systemic skeletal disease characterized by a generalized decrease in bone mineral density that predisposes people to an increased risk of fractures. It is a very common systemic disorder which affects around 300 million people all over the world. This incidence rises with age and is prevalent in females. When there is a diagnosis of osteoporosis the dentist will consider it when planning treatment, modifying the implant geometry, using larger implant diameter and in surface treatment. Osteoporosis is not a contraindication for implant surgery because an accurate analysis of bone quality by means of tomography is performed. The osteoporotic bone is similar to the proposed model of bone type IV, that means it is the softest type of bone.
During an implant placement, the dentist can evaluate the bone density through tactile feedback. Osseointegration in implantology is the process of connecting bone to the implant surface without intervening soft tissue. This is an important factor to consider in patient diagnosis. Another important concept to consider for successful implant placement is the primary implant stability which refers to the stability of a dental implant immediately after implantation. Osteoporosis, which is characterized by bone loss, alteration of the micro structure and the reduction in the regenerative capacity of bone, has been considered a possible contraindication or a risk factor for dental implants placement. Proper oral hygiene prior to intervention is highly advised.
Clinical case:
Patient with Osteoporosis.
The patient had problems with the existing natural teeth and requested a solution for failing mobile teeth. After the assessment it was decided that dental implants would be the best long term solution. As expected the bone tissue was extremely soft. During the implant surgery none of the implants placed had any stability in the jaw bone. A period of 6 months was recommended for healing of the implants in the jaw bone.
